How ARA-290 Was Discovered

ARA-290's origin traces to a 2004 discovery by researchers Michael Brines and Anthony Cerami: that erythropoietin (EPO) — the hormone best known for stimulating red blood cell production — also protects tissue through a completely separate receptor pathway. They identified this second target as the “innate repair receptor,” a heteromeric complex formed by the EPO receptor and CD131, expressed on nerve, glial, and endothelial cells under conditions of stress or injury — distinct from the classic EPO receptor pathway responsible for red blood cell production.

That discovery reframed a longstanding problem: EPO's tissue-protective effects were well documented, but using EPO itself for that purpose meant also triggering unwanted increases in red blood cell counts. Working at Araim Pharmaceuticals, Brines and Cerami's team engineered ARA-290 as an 11-amino-acid fragment of EPO's helix-B surface, deliberately designed to selectively activate the innate repair receptor while avoiding the classic EPO receptor pathway entirely — isolating the tissue-protective signal from the erythropoietic one.

What ARA-290 Has Been Studied For

ARA-290's research reached further into human trials than most compounds referenced on this site. A Phase 2 trial in sarcoidosis-associated small-fiber neuropathy found significant improvements in neuropathic pain scores and corneal nerve fiber density, and a follow-up Phase 2b trial replicated that nerve-regeneration finding. Development stalled after Araim Pharmaceuticals shut down, and no Phase 3 trial was initiated, though ARA-290 holds FDA orphan drug status for neuropathic pain in sarcoidosis.
