What Is Cagrilintide?

Cagrilintide is a synthetic amylin analog developed by Novo Nordisk, using the same fatty-acid acylation technology as semaglutide to extend its half-life for once-weekly subcutaneous dosing. Amylin is a hormone co-secreted with insulin by pancreatic beta cells, and it acts on amylin receptors in the area postrema of the brainstem — a mechanism distinct from, and studied as complementary to, the GLP-1 receptor agonism used by Retatrutide and other incretin-based compounds. Because amylin and GLP-1 signal through separate receptor systems, the two are studied together for potential additive effects on appetite and food intake.

Clinical & Preclinical Trial Data

As monotherapy, a Phase 2 dose-finding trial (Lau et al., The Lancet, 2021; 706 adults with overweight or obesity, 26 weeks, doses from 0.3mg to 4.5mg weekly) found the highest dose produced a mean weight reduction of 10.8%, compared to 3.0% with placebo and 9.0% with liraglutide 3.0mg as an active comparator.

Combined with semaglutide as CagriSema, the Phase 3 REDEFINE program has reported considerably larger effects: in the REDEFINE-1 trial, participants lost an average of approximately 23% of body weight under the trial's treatment-policy estimand (the effect if all participants remained on treatment). Novo Nordisk has filed a New Drug Application (NDA) for CagriSema with the FDA, with a decision expected later in 2026. As of this writing, neither cagrilintide alone nor CagriSema has received FDA approval.
